FS Visitation, Participation Agreement

Please download, complete, and sign for those who will be coming for your scheduled visit. Parent(s) and child(ren) can be on one form if they have the same parent/guardian.  Adults, 18 years or older, who are visiting, participating, or coming to watch someone MUST complete their own form and bring it with them. All areas that need completion will be highlighted on page 1 (Adult/Guardian Information), page 4 (Blue to List Kids and Yellow to List Adults), and page 5 (Blue for Family Form Signature(s) and Yellow for Adult Only Form Signature).
